Insurance
Pelvic Balance Physical Therapy strives to provide the highest quality of health care to our clients. Unfortunately, insurance companies have continued to decrease reimbursement of physical therapy services year after year despite increased cost of living. Pelvic health physical therapy is one-on-one so this impacts our specialty more significantly. Highly skilled pelvic health PT's are typically out-of-network due to this unfortunate medical environment with insurance. In order to provide excellent care and keep the business properly running we are an out-of-network practice with insurance companies. This means that our client's care is the number ONE priority which allows for significantly quicker healing time frames, better outcomes, and organized care that is efficient!
WE ARE REGISTERED as OUT-OF-NETWORK providers with Tricare and Availity (used for majority of insurances) which allows for easier processing of Superbills.
We WILL provide you with a Superbill to allow for out-of-network reimbursement from your insurance provider. Please just ask if you need one :)
We recommend that clients contact their insurance company to find out if they have out-of-network benefits for physical therapy.
QUESTIONS TO ASK INSURANCE COMPANY:
- Do I have out-of-network benefits?
- Do my out-of-network benefits include physical therapy?
- Do I have an out-of-network deductible?
- Once my deducible is meet, what percentage or amount of care is reimbursed?
- What is the effective date for my policy?
- Does my insurance policy follow a calendar year or an effective date year?
- What forms do I need to fill out with my superbill invoice to be properly reimbursed with my out-of-network benefits?
- How do I submit these forms? Online via a specific portal/website, fax, email, or mail?
- Are there any additional requirements needed for reimbursement; such as a referral from my primary care doctor, etc?
Collect specific information and follow exactly as they advice for quicker and more efficient reimbursement.
Cost Of Care With Pelvic Balance Physical Therapy
Initial Evaluation: This appointment is utilized to determine cause of pain and/or dysfunction and to determine an individualized plan of care for long term healing. $350 due the day of the appointment.
***All payment forms accepted. A 5% Discount is given when paid via cash.
After your initial evaluation, a plan of care will be determined. We do not do individual sessions at first and require a commitment to the whole plan of care to ensure success. Over the years we have found this has benefited our client’s best, as inconsistent treatment IS NOT successful. Exceptions are made for individuals who have orthopedic concerns not related to pelvic health or for women who are pregnant due to the changing nature of pregnancy. All individual sessions are $225 for a 60-minute treatment session.
Healing Programs (determined during evaluation)
Call or schedule a FREE consultation with a doctor of PT to discuss what healing program you will likely need. The entire program is one set cost (start to finish) that can be paid up-front during the first treatment appointment OR via a 6-month payment plan.

1. Strength Training: Postpartum Rehab, Urinary Leakage due to cough/laugh/sneeze/lifting/jumping, Bowel Leakage, Pelvic Organ Prolapse, Acute injury
- 5, 60-minute treatment sessions & 8-12 weeks of message support access to therapist to properly manage plan of care.
- Constant availability to doctor of PT via messaging during the entire plan of care time frame to allow for clear communication and changes to the plan of care between appointments.
- Guided Individualized Exercise Program with Tracking Handouts & Videos (paper version or phone app)
- Life-Time Access to the Postpartum Workshop for postpartum women and/or TheHappySIJ Master Plan
For more significant pelvic health concerns 8 sessions over 12-14 weeks will be recommended:
- 8, 60-minute treatment sessions & 12-14 weeks of message support access to therapist to properly manage plan of care.
- Constant availability to doctor of PT via messaging during the entire plan of care time frame to allow for clear communication and changes to the plan of care between appointments.
- Guided Individualized Exercise Program with Tracking Handouts & Videos (paper version or phone app)
- Life-Time Access to the Postpartum Workshop for postpartum women and/or TheHappySIJ Master Plan
2. Persistent Pelvic Pain: Vulvodynia, Dyspareunia, Urinary Urgency and Frequency, Painful Bladder Syndrome (IC), Endometriosis, Pudendal Neuralgia, Complex Pelvic Pain Syndrome, Pelvic Floor Dysfunction, Chronic low back pain, Chronic SIJ pain, Chronic health concerns in general...etc
- 10, 60-minute treatment sessions & 8-10 weeks of message support access to therapist to properly manage plan of care.
- Constant availability to doctor of PT via messaging during the entire plan of care time frame to allow for clear communication and changes to the plan of care between appointments.
- Healing Toolkit: Why Pelvic Pain Hurts Book, Massager, Pelvic Wand, Revagination, Slippery Stuff Lubricant.
- TheHappyV Formula Online Program OR TheHappySIJ Master Plan: lifetime access to all care techniques that will be reviewed during sessions and additional healing education to compliment the In-Person Sessions for maximize success and long term results.

For more significant pelvic health concerns 14 sessions over 12 weeks will be recommended:
- 14, 60-minute treatment sessions & 12-14 weeks of message support access to therapist to properly manage plan of care.

Cancellation Policy
Appointments can be canceled easily by calling or texting 813-575-0427 OR by using the scheduling link in your appointment email confirmation.
We will always try to be as accommodating as possible with any schedule changes and politely ask that more than 24 hours notice is given when possible.
As a small business we are significantly affected by late cancelations. We understand that life happens and last minute cancelations are unavoidable at times. We ask for understanding that when a late cancel of less than 24 hours or a missed appointments occurs there will be a cancelation fee of $115. This is not designed to negatively impact you but merely an important boundary our business has to set with clients due to the negative impact it has on the business and schedule for other clients at the practice.
